A forklift operator was placing Progressive Cavity Pumps (PCPs) 5.25m long and weighing 170kg into racking in a designated storage shed. The forklift operator placed a third layer of PCP tubulars on the cantilever racking and started to reverse. The heel of the forklift tine caught a PCP on the 2nd layer and pulled one end of it to the ground (approx. 1.2m). The other end of the PCP then dropped to the ground. A Verification of Competency assessor was observing from about 5m away.
